It would look better If I added the page_background image of the catalogue that apears at the top of the form to the infobox but I was having trouble geting an image to apear and link to the form correctly as I'm a novice at this thing.

I needed a new affiliate infobox, but didn't see a quick a simple contribution that added this without a ton of other stuff. So I created this quick and easy contribution in hopes that it would help others.
