Implementation is very simple using the Forms API, and the checkbox itself is rendered via Javascript when the page loads (and excluded for users who have opted not to enable the checkall module). The site administrators can enable on the settings page a checkall box for all forms on the site.InstallationUnpack in your modules folder (usually /sites/all/modules/) and enable under Administer > Site Building > Modules.
